Adult Community Inclusion Support Specialist

Hilton Community Services Ltd is seeking a dedicated and compassionate Adult Community Inclusion Support Worker to join our team.
About the Role:
We are passionate about ensuring that the people we support make their own choices and live fulfilling lives. As an Adult Community Inclusion Support Worker, you will play a vital role in providing high-quality, person-centred support to adults with learning disabilities, some of whom may have associated disabilities such as physical disabilities, sensory impairment, and mental health needs.
Key Responsibilities:
- Provide individualised support to adults with learning disabilities, promoting their independence and autonomy.
- Develop and implement person-centred support plans, working closely with the individual, their family, and other professionals.
- Support individuals to access community resources, services, and activities, promoting social inclusion and participation.
- Collaborate with other professionals, including healthcare and social care teams, to ensure comprehensive support.
- Contribute to the development and implementation of policies and procedures, ensuring a safe and supportive environment.
Requirements:
- Experience working with adults with learning disabilities and associated needs.
- Strong communication and interpersonal skills, with the ability to build rapport with individuals and their families.
- Ability to work independently and as part of a team, with a flexible approach to working hours.
- Knowledge of relevant legislation, policies, and procedures, including the Mental Capacity Act and the Care Act.
