About This Opportunity

Boston Medical Center is seeking a skilled Adult Care Coordinator to join our team. In this role, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.

The estimated salary for this role is $65,000 - $85,000 per year, depending on experience. You will work closely with our interdisciplinary care teams to ensure the seamless coordination of patient care, providing exceptional customer service and supporting the delivery of high-quality patient care.

Responsibilities:

  1. Coordinate patient care per the NCQA Patient-Centered Medical Home (PCMH) guidelines.
  2. Arrange and manage patient care as directed by the primary care provider.
  3. Provide self-care literature and arrange community support.
  4. Track and coordinate patient care utilization.
  5. Manage patient education room.
  6. Assist patients with self-motivation efforts to help patients manage their care.
  7. Distribute appropriate teaching materials to patient/family to review.
  8. Communicate and assist RN with patient care plans.
  9. Attend morning daily huddles to coordinate patient care.
  10. Coordinate referrals and resources, including Diabetes Center, VNA referrals, DME, meals on wheels.
  11. Assist with completing and tracking patient forms and letters.
  12. Schedule, track and reconcile patient referrals and diagnostic tests, as appropriate.
  13. Coordinate care transitions.
  14. Assist with tracking quality and preventive screening measures.
  15. Assist with patient outreach.
  16. Perform other duties as assigned.
  17. Schedule test per written SEHC workflow with SEMC Central Scheduler via telephone or electronically.
  18. Coordinate patient flow throughout all scheduled departments.
  19. Notify SEMC Central Scheduler when Interpreter services are needed.
  20. Answer telephones, route calls, take messages, and provide information to callers in a polite and professional manner per established RN and ACC telephone trees.
  21. Keep up with changes in departmental hospital regulations, policies, and procedures.
  22. Show respect for patient privacy by following established HIPAA guidelines.
  23. Follow Joint Commission policies and procedures, including the National Patient Safety Goals.
  24. Cross train to other areas of the Health Center, as appropriate.
  25. Process Prescription Prior Authorizations.
  26. Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
  27. Commit to recognize and respect cultural diversity for all customers (internal and external).
  28. Communicate effectively with internal and external customers with respect of differences in cultures, values, beliefs and ages, utilizing interpreters when needed.
