Job Title: CPN - Care Coordinator Early Intervention in Psychosis

Description:

We are seeking a skilled CPN to join our Mental Health Team as a Care Coordinator for Early Intervention in Psychosis. The successful candidate will work closely with our multidisciplinary team to provide comprehensive care and support to individuals experiencing their first episode of psychosis or at-risk mental state.

Key Responsibilities:

  • Provide specialist assessment, planning, and intervention to individuals with first episode psychosis or at-risk mental state.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
  • Conduct regular reviews and evaluations to ensure the effectiveness of care plans and make recommendations for improvement.
  • Develop and maintain strong relationships with service users, carers, and other professionals to ensure a comprehensive and coordinated approach to care.

Person Specification:

  • NMC/HCPC registered
  • Postgraduate clinical experience
  • Strong interest in working with individuals experiencing psychosis
  • Desirable: community mental health experience and experience of working with families
