East Durham Community Health Navigator

We are seeking a highly skilled East Durham Community Health Navigator to join our team at Tewv Brits. As a key member of the primary care network, you will play a vital role in facilitating communication between healthcare professionals and patients. Your responsibilities will include providing clinical supervision, teaching, training, and assessing in clinical practice. You will also contribute to quality improvement activities and help promote ease of access to specialist mental health support services.
Requirements- A current professional registration with the NMC.
- NMP Level 1 qualification (or willing to obtain within 12 months).
- Experience working with people with mental ill health in a community setting.
