Job Description

We are seeking a passionate and dedicated Field Care Manager to oversee the delivery of high-quality home care services. The successful candidate will work closely with clients, their families, and healthcare professionals to ensure holistic care.

Key Responsibilities:

  • Care Planning & Assessment: Develop and review personalized care plans, ensuring compliance with CQC regulations.
  • Liaising with the Operational Team: Addressing performance issues, providing training, and ensuring seamless service delivery.
  • Quality Assurance: Conducting audits, maintaining accurate records, and ensuring services meet CQC standards.
  • Building Relationships: Establishing trust and confidence with clients, their families, and healthcare professionals.

Requirements:

  • NVQ Level 3 or 4 in Health & Social Care (or equivalent).
  • Experience in a supervisory/management role within health and social care.
  • A passion for delivering high-quality care, exceptional communication, and organizational skills.
  • Full UK driving license and ability to travel using own car.

Benefits:

  • Competitive salary (£27,000 per annum).
  • Remote/field-based role.
  • Flexible working hours.
  • Mileage paid at 45p per mile.
  • Annual leave - 25 days plus bank holidays.

Estimated Salary: £27,000 per annum.
