We are seeking an experienced Community Care Specialist to join our Urgent Community Response service in Warrington. This is a challenging and rewarding role that requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care.
The successful candidate will have the opportunity to work with a multidisciplinary team, providing expert care and support to patients in their homes across Warrington Borough. Our aim is to prevent hospital admissions and support timely hospital discharges, ensuring the best possible outcomes for our patients.
Job DescriptionThis is a dynamic role that requires strong leadership skills, excellent communication abilities, and a passion for delivering high-quality patient care. As a key member of our team, you will be responsible for promoting effective multidisciplinary patient care, working closely with colleagues across the wider health and social care environment.
You will also contribute to the assessment, diagnosis, care, and treatment of patients at home, preventing unnecessary hospital admission. The ideal candidate will have relevant experience in Urgent care and/or primary care, with extensive clinical skills and knowledge.
Requirements- Nurse registration and current registration.
- Mentorship Qualification.
- Advanced Clinical Skills Modules at Masters Level (Level 7) Clinical examination and Clinical diagnostics.
- V300 Non-medical prescribing or willingness to work towards.
- Relevant and current experience in Urgent care and/or primary care with extensive clinical skills and knowledge.
- Evidence of continuing professional development.
- Proficient IT Skills.
