About the Job
We are seeking a skilled and dedicated Registered Nurse to join our Reactive Emergency Assessment Community Team (REACT) as a key member of our multidisciplinary team. The successful candidate will be responsible for delivering excellent patient-focused care to patients in the East Suffolk community.
The role involves traveling throughout East Suffolk to deliver acute care in patients' homes as part of the REACT nursing team. You will work in an internal rotation, with a mix of core hours, out-of-hours, early shifts, late and night shifts, including weekends and bank holidays. During out-of-hours, you will be accompanied by a Health Care Assistant.
Main Responsibilities
- To assess, plan, and deliver healthcare for patients with long-term conditions, palliative care, and rehabilitation needs to achieve quality of life and independence where possible.
- To work with the multidisciplinary team to deliver Outpatient Parenteral Antibiotic Therapy (OPAT) in the community to facilitate discharge from hospital or prevent unnecessary admission to hospital.
- To work within the integrated team to prevent unnecessary admission to hospital.
- To work with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
