We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at Norfolk and Suffolk NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care and support to individuals with mental health needs in the community.

Key Responsibilities:

  • Manage a caseload of patients with mental health needs, providing planned care and risk assessments.
  • Work closely with the multidisciplinary team to ensure continuity of practice and effective care delivery.
  • Participate in the duty worker rota, providing support and coverage to the team.
  • Supervise students on placement within the team, providing guidance and mentorship.

Requirements:

  • Qualified Registered Mental Health Nurse with experience in community mental health.
  • Strong understanding of the role as a Care Coordinator/Assessor.
  • Ability to work in a dynamic and fast-paced environment.
  • Excellent communication and interpersonal skills.
