Job Description

We are seeking a highly motivated and enthusiastic pharmacist to join our team as a Clinical Care Specialist in Hospital at Home. As part of this role, you will be responsible for providing holistic patient-centered care to improve the safety and effectiveness of individuals' medicines.

The ideal candidate will have gained postgraduate experience working in a patient-focused clinical environment and will have excellent interpersonal and communication skills. You will also demonstrate leadership and innovation consistent with NHS Scotland's values of quality, teamwork, care, and compassion, dignity, and respect, and openness, honesty, and responsibility.

Key Responsibilities:

  • Providing direct pharmaceutical care to older people with complex needs in primary care and community hospitals
  • Liaising with secondary care acute and St Johns Hospital
  • Working with hospital at home and other teams in West Lothian where appropriate
  • Provision and support of pharmaceutical care initiatives in primary care in co-operation with primary care, social care, and community pharmacies
