About the Role

We are seeking a highly skilled and motivated Clinical Pharmacy Service Lead to join our team at Central London Community Healthcare Trust. As a key member of our Medicines Management team, you will be responsible for providing a high-quality specialist clinical pharmacy service across a range of inpatient settings in our community hospitals.

Main Duties

  • Develop and implement effective medicines management initiatives to reduce drug waste and improve patient flow.
  • Provide support and guidance to Medicine Management Pharmacy Technicians.
  • Lead on individualised patient pharmaceutical care, prioritising patients on a day-to-day basis at ward level.
  • Facilitate changes to patients' own use of medicines to improve safety, adherence, and independence.
  • Participate in ward rounds and multidisciplinary teams to ensure continuity of pharmaceutical care.

About You

We are looking for an experienced pharmacist with excellent communication skills, a strong understanding of prescribing and pharmacy issues, and the ability to manage workload effectively. You should have experience of working within multi-disciplinary groups and structured medication review. Postgraduate Clinical Pharmacy Certificate qualification or equivalent is desirable.
