Learning Disability Care Coordinator

We are seeking an experienced Learning Disability Care Coordinator to join our team and provide support to individuals with complex needs.

The successful candidate will possess a strong background in learning disability care, with experience in coordinating services that cater to individuals with autism and other complex conditions.

You will be responsible for ensuring the highest standards of care are maintained at all times, and for ensuring compliance with relevant legislation and regulations.

Key Responsibilities:

  • Coordinate care services for individuals with complex needs
  • Develop and implement effective care plans to meet the unique needs of each individual
  • Maintain excellent relationships with service users, their families, and other stakeholders
  • Ensure the service remains compliant with relevant laws and regulations

Requirements:

  • Degree-level qualification in a relevant field (e.g. social work, nursing)
  • Experience in coordinating services for individuals with complex needs
  • Strong knowledge of relevant legislation and regulations (e.g. CQC, safeguarding)
  • Excellent communication and organisational skills

Salary: £37,000 per annum

Location: East Sussex
