Main Responsibilities:
  • Conduct thorough assessments and develop personalized care plans
  • Implement evidence-based interventions to enhance patient wellbeing and quality of life
  • Maintain accurate and comprehensive clinical records
Working Conditions:

This position involves working in various settings, including in-patient units, patients' homes, and day services. A clean driving licence and reliable transportation are required. We also require vaccination history or evidence of immunity.

A minimum of 3 years post-graduate experience and excellent communication skills are essential for this role, allowing you to effectively collaborate with multidisciplinary teams and communicate with patients' families and carers.
