About the Role:
We are seeking a skilled and compassionate Complex Needs Navigator to join our team at Change Grow Live. As a key member of our outreach team, you will play a vital role in engaging with service users who are experiencing complex needs, including substance misuse and mental health issues.
Your Key Responsibilities:
- Coordinate and lead solution-based approaches to support service users in moving away from street-based living.
- Identify and establish contact with rough sleepers and beggars, rapidly assisting service users through street outreach shifts at various times and locations.
- Carry a caseload of complex clients, developing personalized action plans to discourage rough sleeping.
- Participate in multi-agency initiatives to reduce street activity.
- Work closely with drug and alcohol teams to ensure substance misusers and problem drinkers access treatment.
- Ensure service users have access to ongoing healthcare.
- Participate in statistical analysis of rough sleepers and street counts as required.
About You:
We are looking for an experienced professional with a background in substance misuse, alcohol misuse, or mental health. You will have a proven track record of working with people who have multiple and complex needs, as well as experience of multi-agency working and facilitating meetings.
