About the Role:
We are seeking a highly motivated and clinically-experienced registered clinician to join our newly established Learning Disability and Autism (LDA) Outreach Team at Devon Partnership NHS Trust. As a Clinical Lead, you will work in collaboration with other LDA Outreach colleagues, community learning disability and autism colleagues, and colleagues from 'The Brook', our 10-bedded Learning Disability and Autism inpatient unit.

The successful candidate will have a broad multidisciplinary background, including Mental Health and Learning Disability nursing, Occupational Therapy, and Speech and Language Therapy. They will be responsible for triaging, assessing, planning, and delivering complex care packages to admission-vulnerable service users across Devon and those discharged from The Brook, ensuring proactive liaison with other partner agencies, families, and care providers at every stage of care.
