We are looking for a compassionate and skilled Community Link Worker Lead to join our team in Fife. As a Post Diagnostic Support Link Worker, you will play a vital role in supporting individuals living with dementia and their families to live well in their community.

In this dynamic role, you will work closely with the Community Mental Health Team at Queen Margaret Hospital in Dunfermline to deliver high-quality support services. Your exceptional communication skills and ability to build strong relationships will be essential in connecting people with the community and promoting independent living.

The estimated salary for this position is £26,000 - £32,000 per annum, depending on experience. In addition to a competitive salary, you will have access to ongoing training and development opportunities to enhance your skills and knowledge in dementia care.

Key responsibilities include:

  • Developing and implementing individualised support plans to meet the unique needs of each person
  • Facilitating group sessions and workshops to promote independence and community engagement
  • Collaborating with healthcare professionals to ensure seamless care transitions
  • Maintaining accurate records and reporting on service outcomes
