Children's Healthcare Coordinator

Salary: £45,000 - £50,000 per annum, plus a competitive benefits package including up to 2 days working from home per week, 33 days holiday, and a private pension scheme.
Responsibilities:
* Develop and implement personalized care plans that meet the unique needs of each client
* Lead and manage multidisciplinary teams, ensuring effective communication and collaboration
* Conduct ongoing assessments and evaluations of clients' physical and physiological needs, providing timely support and interventions as needed
* Coordinate the delivery of nursing interventions, including medicine administration and tracheostomy care
* Establish and maintain positive relationships with clients' families, providing emotional support and guidance
* Collaborate with community teams, CHC nurses, and CCG commissioners to design and deliver bespoke packages of care that meet the needs of each client
Requirements:
* RSCN qualification with at least 6 months' experience in the UK and an active NMC PIN
* Proven leadership and management skills, with a focus on empowering colleagues to achieve their best
* Recent experience of tracheostomy care and ventilator management within the last 2 years
* Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
* Suitable for Band 5/6 Nurses with a track record of successful leadership and management of HCAs
* Full UK driving license desirable but not essential
