Outpatient Care Coordinator

We are looking for an enthusiastic and energetic professional to join our team as an Outpatient Care Coordinator. You will be responsible for assessing patients' health and wellbeing across a complex and changing caseload and planning, delivering and evaluating care to meet patients' health and wellbeing needs.
The salary for this role is around £40,000 - £60,000 per annum, depending on experience. You must have experience in Minor Ops and Wound dressings required, and be able to work a variety of shift patterns.
