Community Health Care Specialist

We are seeking a dynamic and passionate nurse to join our Community Nursing Team as a Team Leader in Falmouth/Penryn. The successful candidate will work closely with the team to ensure high-quality, safe, evidence-based care is provided to patients and carers, aiming to maintain them in their own homes.
The ideal candidate will be a calm, caring, and compassionate clinician with an interest in delivering the highest quality care and supporting patients to lead full and independent lives while promoting self-management of their health conditions.
Main Duties- As an autonomous practitioner, you will be responsible for assessing and treating patients with complex health and social care needs.
- You will manage a clinical caseload within a community nursing team, working closely with other healthcare professionals to provide comprehensive care.
The estimated salary for this role is £42,049 - £62,476 per annum, depending on experience.
