Job Title: Experienced Skilled Home Healthcare Professional

Cape Cod Healthcare is seeking a skilled and experienced home health nurse to join our team. In this role, you will be responsible for delivering high-quality patient-centered care to assigned patients.

Key Responsibilities:

  • Update the Clinical Manager on referrals to community resources as needed
  • Instruct and supervise Home Health Aides in accordance with established plans of care every 14 days
  • Notify physicians and other involved care providers of significant patient condition changes
  • Record clear, concise, and complete narrative documentation

Our ideal candidate has a minimum of one year of nursing experience in home health care or six months of experience with a BSN degree. Proficiency in computer skills and IV therapy is highly desirable.

Benefits:

  • Minimum one (1) year of nursing experience required or 6 months of nursing experience with a BSN
  • Home care and IV experience, computer skills, and BSN preferred
  • Estimated Salary Range: $65,000 - $85,000 per year
