At Phoenix MCT, we're seeking a highly skilled Dementia Care Specialist to join our team in Cornwall and the Isles of Scilly. This is an exciting opportunity to work with people who have dementia and their families, providing expert care and support throughout their journey.

The estimated salary for this role is £38,000 - £42,000 per annum, depending on experience. As a Primary Care Dementia Practitioner, you will work closely with GP surgeries, holding a case list of people diagnosed with dementia and registered to those surgeries.

This is a particularly rewarding time for us as we're embarking on a joint venture between Alzheimer's Society, Disability Cornwall, and Memory Matters, strengthening our collaborative working to make a positive difference for people with dementia, their families, and carers.

Your key responsibilities will include:
  1. Conducting skilled assessments to identify the needs of people with dementia and their family carers.
  2. Providing specialist information and advice for carers on various aspects of caring for a relative or friend with dementia.
  3. Working with people and their families from diagnosis throughout their dementia journey, offering emotional, psychological support, and guidance on accessing services.
  4. Helping family carers develop and improve their skills to assist with caregiving.
  5. Promoting positive approaches to living well with dementia.
  6. Ensuring the person with dementia is at the centre of everything we do.
  7. Collaborating with the Memory Assessment Service and Dementia Older People's Mental Health Service to provide a comprehensive and seamless service across the pathway.
  8. Working alongside other professionals and organizations to ensure coordinated care.
  9. Improving GP awareness, assessment, and diagnosis of people with dementia and identifying those on the GP dementia registers.
  10. Having access to supervision and appraisal to develop your expertise and support the service in meeting the needs of the local community effectively.
