We are currently seeking a highly motivated and experienced Senior Primary Care Dementia Practitioner to join our team in the Mid locality of Cornwall.

About the Opportunity

This is an excellent opportunity to work as part of a dynamic multidisciplinary team, providing specialist support to individuals with dementia and their families.

As a key member of the team, you will be responsible for managing a caseload of clients, providing individualized care and support, and working closely with GPs, practice nurses, and other healthcare professionals to deliver high-quality patient care.

The ideal candidate will have substantial experience of managing a busy caseload, with a proven track record of delivering high-quality patient care in a primary care setting.
