Cornwall Partnership NHS Foundation Trust is seeking a Community Care Coordinator to join our team. This is an exciting opportunity for a skilled professional to work with individuals living with dementia and mental health needs, providing assessment, treatment, and care coordination.
Main Responsibilities
- Providing clinical supervision and managing a busy caseload of clients with complex mental health issues.
- Working closely with statutory and non-statutory agencies to develop and deliver personalized care under the Care Programme Approach.
Requirements
- Registered Mental Health Nurse (RMN) with postgraduate qualifications in psychological therapy.
- Substantial experience in managing complex mental health cases and providing clinical supervision.
Salary and Benefits
The salary for this role is approximately £48,000 per annum, plus benefits including pension scheme, annual leave, and opportunities for career development.
