Paediatric Case Coordinator

In this role, you will work closely with our multidisciplinary team to coordinate high-quality care services for paediatric patients. As a Paediatric Case Coordinator, you will be responsible for designing and delivering tailored care plans that meet the unique needs of each client.
The successful candidate will have the opportunity to build strong relationships with clients and their families, providing emotional support and guidance throughout the care process.
Key Responsibilities:- Developing and implementing individualised care strategies that address the physical and psychological needs of clients
- Collaborating with the multidisciplinary team to ensure seamless care delivery and optimal patient outcomes
- Providing ongoing monitoring, assessment, and evaluation of clients' needs, making adjustments to care plans as necessary
- Establishing and maintaining strong relationships with clients and family members
- Working collaboratively with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
- RSCN with a minimum of 2 years' experience in the UK and an active NMC PIN
- A supportive management style that fosters growth and development among team members
- Experience with tracheostomy care and ventilators
- Advanced communication skills, both verbal and written, to liaise effectively with patients, families, and the multidisciplinary team
- A full UK driving licence is essential
- An estimated salary of 49,000 per annum (dependent on experience)
