Support Practitioner Role in Community Support

We are seeking a highly motivated and compassionate individual to join our team as a Support Practitioner. As a key member of our team, you will provide essential support to individuals with complex needs, promoting their independence and well-being.
Key Responsibilities:- Provide social support by assisting individuals to engage with their communities through leisure activities such as day centres, cinema, swimming, etc.
- Administer and monitor medication according to individual needs.
- Offer personal care by promoting individuals' wellbeing, including assistance with bathing, dressing, arranging visits with dentists/doctors, and incontinence care.
- Offer practical support by helping individuals maintain their tenancy, encouraging daily household tasks such as general housework, laundry, ironing, cooking, shopping.
To be successful in this role, you will need to have a full manual driving licence, as you will be required to drive supported individuals' mobility cars. You must also be flexible, as shifts vary from early, back, late, and sleepover. Additionally, you will be required to register with the SSSC within 3 months of your start date.

The estimated salary for this position is £25,000 - £30,000 per annum, depending on experience.
