Role Overview

We are currently recruiting for a Discharge Lounge Team Member to join our team at Maidstone and Tunbridge Wells NHS Trust.

As a Nurse, you will be responsible for ensuring the smooth transition of patients from hospital to home, working closely with inpatient teams, medical and surgical teams, and external providers.

Main Responsibilities
  • Coordinate the discharge process, including documentation and reporting
  • Develop and maintain relationships with inpatient teams to ensure effective communication and collaboration
  • Work collaboratively with the Care Coordination Centre, Site Directors, and Site Practitioners to ensure a safe and effective flow
