Job Summary

Cera Care is seeking a skilled Physiotherapist to join our team in providing high-quality care to adults in their own homes. As a Physiotherapist - Community Health Specialist, you will be responsible for assessing and treating patients, providing advice and guidance to support workers and health professionals, and working collaboratively with other teams to achieve personalized goals.

Key Responsibilities

  • Assess and treat patients in their own homes, developing and implementing personalized treatment plans to promote independence and well-being.
  • Provide expert advice and guidance to support workers and health professionals, ensuring seamless care coordination and effective communication.
  • Work collaboratively with other teams, including social care and healthcare professionals, to achieve personalized goals and promote holistic care.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to individual needs and preferences.
  • Monitor and evaluate treatment progress, making adjustments as needed to ensure optimal outcomes.

Requirements

  • Professional Qualification in Physiotherapy
  • HPC Registration
  • 2 years experience working as an autonomous Physiotherapist
  • Experience in working in the Community
  • Experience of team working and liaison with other staff groups
  • Evidence of CPD portfolio
  • Awareness and understanding of NICE guidelines

Desirable Experience

  • Working in a Reablement environment to achieve personalized goals
  • Working with a wide range of service users with varying conditions and needs
