Job Title: Community Physiotherapist

Norfolk Community Health and Care NHS Trust is seeking a skilled and dedicated Community Physiotherapist to join our team. As a Community Physiotherapist, you will play a vital role in delivering high-quality patient care and rehabilitation services to individuals in their own homes, care homes, and community settings.

Job Summary

The successful candidate will be responsible for assessing, planning, implementing, and evaluating packages of care for patients with a range of physical and functional needs. You will work collaboratively with multidisciplinary teams to provide patient-centered care and support individuals to achieve their goals and regain independence.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their physical and functional needs.
  • Develop and implement individualized treatment plans to address patient goals and objectives.
  • Provide evidence-based physiotherapy interventions to promote patient recovery and independence.
  • Collaborate with healthcare professionals and community teams to ensure seamless care coordination and patient outcomes.
  • Monitor and evaluate patient progress, making adjustments to treatment plans as needed.
  • Communicate effectively with patients, families, and healthcare professionals to ensure patient-centered care.
  • Participate in multidisciplinary team meetings and contribute to the development of care plans.
Requirements
  • BSc Hons in Physiotherapy or equivalent qualification.
  • Active HCPC registration.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and analytical skills.
  • Ability to adapt to changing priorities and deadlines.
