Job Title: Community Physiotherapist

Norfolk Community Health and Care NHS Trust is seeking a skilled and dedicated Community Physiotherapist to join our team. As a Community Physiotherapist, you will play a vital role in delivering high-quality patient care and rehabilitation services to individuals in their own homes, care homes, and community settings.

Job Summary

The successful candidate will be responsible for assessing, planning, implementing, and evaluating packages of care for patients with a range of physical and functional needs. You will work collaboratively with multidisciplinary teams to provide patient-centered care and support patients to achieve their goals and regain independence.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their physical and functional needs.
  • Develop and implement individualized treatment plans to address patient needs and goals.
  • Work collaboratively with patients, families, and healthcare teams to provide patient-centered care and support.
  • Provide education and training to patients and their families on self-management techniques and strategies.
  • Contribute to the development and implementation of rehabilitation programs and services.
  • Participate in multidisciplinary team meetings and contribute to the development of patient care plans.
  • Maintain accurate and up-to-date patient records and reports.
Requirements
  • BSc Hons in Physiotherapy or equivalent qualification.
  • Active HCPC registration.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and analytical skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
