Job Title: Physiotherapist - Community Health Specialist
Job Summary:
We are seeking a highly skilled Physiotherapist to join our team at Cera Care. As a Physiotherapist - Community Health Specialist, you will be responsible for assessing and treating adults in their own homes during their Reablement journey.
Key Responsibilities:
- Assess, plan, implement, and monitor physiotherapy treatment programmes for Reablement adults within their own homes.
- Analyse, evaluate, and modify treatment to measure progress and assure effectiveness of the intervention.
- Be professionally and legally accountable for all aspects of your own work to ensure a high standard of clinical care.
- Refer and escalate to other staff and agencies within Cera and key external stakeholders as appropriate.
- Assist in relevant aspects of health promotion for service users within the Reablement pathway.
- Refer to and recommend the provision of aids to assist the service user to achieve their reablement goals.
- Establish effective communication networks with service users, support workers, and other team members.
- Meet company set targets.
- Be responsible for the effective and efficient organisation of your own time management, prioritisation, workload, and the safe use of equipment.
- Adhere to Cera's rights-based approach, taking into account service user choice.
- Take reasonable care for ensuring your own personal health and safety and that of service users and other staff that may be affected by your acts or omissions.
Professional Requirements:
- Comply with and adhere to the Health Professions Council Standards of Professional Conduct, the Chartered Society of Physiotherapy's Standards of Physiotherapy Practice, and Trust and local Policies and Procedures.
- Set a high standard of personal practice and encourage others to do likewise and to have a commitment to lifelong learning and development of a relevant CPD portfolio as evidence of this.
- Have responsibility towards Safeguarding all customers and staff escalating concerns immediately to the line manager.
- Respect the individuality, values, culture, and religious diversity of service users and contribute to the provision of and evaluation of a service sensitive to these needs.
- Represent the physiotherapy service at meetings and represent Cera.
- Continually review and evaluate the evidence base of practice through research or published papers and apply to own practice and implement change to working practices as is appropriate and agreed.
- Review and reflect on your own practice and performance through effective use of professional and operational supervision and appraisal.
- Actively participate in an annual individual Performance Review and development of a Personal Development Plan to apply acquired skills and knowledge in professional practice.
Qualifications and Experience:
- Professional Qualification in Physiotherapy.
- HPC Registration.
- 2 years experience working as an autonomous Physiotherapist.
- Experience in working in the Community.
- Experience of team working and liaison with other staff groups.
- Evidence of CPD portfolio.
- Awareness and understanding of NICE guidelines.
