We are looking for an exceptional Care Management Support Specialist to join our team

Job Description:
  • Support the Service Manager: Collaborate with the Service Manager to ensure a safe service environment for individuals we support and colleagues.
  • Compliance and Quality: Ensure adherence to legislation, Cartrefi Cymru Co policies, procedures, and quality standards.
  • Person-Centred Support: Deliver high-quality, person-centred support to service users, promoting their independence and well-being.

Requirements:
  • Experience: Supporting adults with learning disabilities in social care settings.
  • Qualifications: QCF Level 3 in Health and Social Care.
  • Driving License: Possession of a full clean driving license.

Estimated Salary: £40,000 - £45,000 per year
