Job Title: Band 5/6 Practice Nurse

We are seeking a highly skilled and experienced Practice Nurse to join our team at the NHS. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Main Responsibilities:
  • Develop and implement health promotion and well-being programs to improve patient outcomes.
  • Provide routine nursing care to patients, including wound care, venepuncture, and ECGs.
  • Manage and support patients with chronic diseases, such as diabetes, asthma, and CHD.
  • Conduct new patient medicals and provide health advice to patients.
  • Request pathology tests and process results, advising patients accordingly.
Secondary Responsibilities:
  • Act as an audit lead, utilizing the audit cycle to improve practice performance.
  • Monitor and ensure the safe storage, rotation, and disposal of medicaments.
  • Support junior members of the nursing team, providing guidance and mentorship.
  • Participate in local initiatives to enhance service delivery and patient care.
Person Specification:
  • Registered Nurse with a postgraduate diploma or degree in Primary Care.
  • Experience working in a primary care environment, preferably as a Practice Nurse or Community Nurse.
  • Strong clinical knowledge, including wound care, ECGs, venepuncture, and chronic disease management.
