About the Role

This exciting opportunity is a key part of our RECONNECT initiative, a pioneering NHS England project aimed at reducing reoffending rates among high-risk individuals with complex health needs.

Key Responsibilities

  • Collaborate with cross-agency teams to provide coordinated intervention, support, and risk management for individuals leaving prison.
  • Apply clinical expertise to develop and implement a trauma-informed, formulation-driven approach to support service users.
  • Forge new care pathways in the community, working with a range of non-NHS providers to deliver innovative substance misuse interventions.
  • Partner with local authorities, health services, and voluntary sector organizations to ensure seamless support for individuals with multiple needs.

Requirements

  • Extensive experience in forensic mental health settings, including community, inpatient services, and prisons.
  • Strong knowledge of recovery-based models within drug/alcohol use and the importance of service user involvement.
  • Excellent communication and partnership working skills, with experience of collaborating across health, social care, and criminal justice agencies.
