Discharge Support Liaison Nurse

An exciting opportunity has arisen for a motivated and suitably experienced professional to join Raigmore Hospital's Discharge Support Team.
The team is central to efficient patient flow from the acute hospital, and the successful candidate will enjoy being part of a dynamic and busy team where multidisciplinary working and effective communication is key.
Significant and relevant experience in supporting complex patient journeys is necessary, as well as excellent IT skills. An awareness of local health and social care priorities is advantageous with a patient-centred approach to care being essential to this role.
Only those with a current NMC PIN will be considered for short-listing.
