About the Role
  • The successful candidate will be responsible for coordinating patient care, developing care plans, and leading a team of healthcare professionals.
  • They will also provide mentorship to students and Healthcare Assistants, ensuring they receive the best possible training and support.

The ideal candidate will have experience working in a community hospital setting, with a strong focus on care of the elderly. They will be able to work effectively as part of a team, communicate clearly with colleagues and patients, and demonstrate a passion for delivering exceptional patient care.

Main Responsibilities
  1. To coordinate patient care, developing individualized care plans and ensuring effective communication among healthcare professionals.
  2. To lead a team of healthcare professionals, providing guidance and support as needed.
  3. To provide mentorship to students and Healthcare Assistants, ensuring they receive comprehensive training and development opportunities.
Requirements
  • Evidence of ongoing professional development and commitment to continuous learning.
  • Current professional registration with the NMC and experience working within a community hospital setting.
  • Ability to work effectively as part of a team, communicating clearly with colleagues and patients.

This is a Band 5 post, attracting an annual salary of £29,970 - £36,483, depending on experience.
