Community Support Specialist

We are seeking a dedicated and caring Community Support Specialist to join our team at CLICK Primary Care Network. As a key member of our team, you will work closely with our healthcare professionals to deliver high-quality patient care and support in the community.
Key Responsibilities- Providing patient care and support in the community, working as part of a wider health and social care team.
- Managing your own workload and case load effectively, prioritizing tasks and deadlines as necessary.
- Working collaboratively with patients, relatives, carers, and other healthcare professionals to ensure clear lines of communication and effective care delivery.
The ideal candidate will have a strong foundation in phlebotomy, ECG/ECG holter tests, weight, ambulatory blood pressure monitoring, and other essential clinical interventions. They will also possess excellent analytical and problem-solving skills, with a basic understanding of the principles and philosophy of health and social care.
In return, we offer a competitive salary of £35,000-£45,000 per annum, depending on experience, plus benefits including annual leave, pension scheme, and professional development opportunities.
