We are seeking an experienced Adult Social Worker to join our Disability South Team in Croydon. As a key member of our team, you will be responsible for assessing and providing support to adults with disabilities, ensuring they receive the best possible care and services.
Key Responsibilities:
- Conduct thorough assessments of adults with disabilities to identify their needs and develop personalized care plans
- Work closely with service users, their families, and other professionals to ensure seamless delivery of services
- Collaborate with internal and external stakeholders to ensure effective service coordination and communication
- Provide guidance and support to colleagues to ensure high-quality service delivery
