About the Role

The Senior Care Coordinator position at Superior Healthcare presents an exciting opportunity to join a dynamic team dedicated to providing high-quality care to clients with long-term health conditions.

As a key member of our Complex Care Division, you will be responsible for managing client caseloads, collaborating with clients, their families, and carers, and ensuring seamless coordination of care services.

We are seeking an individual with previous care coordination experience, excellent interpersonal skills, and a genuine interest in working within a caring environment.

The successful candidate will be able to work autonomously and as part of a wider team, maintaining high standards in communication and event management.

Key Responsibilities
  • Manage client caseloads and ensure effective coordination of care services.
  • Collaborate with clients, their families, and carers to understand their needs and preferences.
  • Proactively address day-to-day events and maintain high standards in communication and event management.
  • Implement and maintain Superior Healthcare's quality assurance programme, ensuring compliance with policies and safeguarding procedures.
Requirements
  • Previous care coordination experience.
  • Excellent interpersonal skills and a genuine interest in working within a caring environment.
  • Ability to work autonomously and as part of a wider team.
  • Observant and sensitive to carers' and clients' needs.
  • Excellent oral and written communication skills.
  • Good IT skills, including use of Microsoft Teams and Outlook.
