About the Role

We are seeking a Support Worker to join our Children and Young People's Short Breaks service in Clevedon. As a Support Worker, you will play a vital role in empowering young people to live the lives they want to lead.

Key Responsibilities

  • To develop daily living skills and support individuals to access their communities for social activities, employment, training, or friendships.
  • To contribute to, develop, and implement individual care plans.
  • To communicate effectively and professionally with individuals, their family members, and external professionals.
  • To maintain the health needs of the individuals we support.
