Cancer Patient Care Navigator

We are seeking a compassionate and experienced Cancer Patient Care Navigator to join our team at Lewisham and Greenwich NHS Trust. This role offers a chance to provide holistic support to cancer patients and their families.
About the Position
- This is a full-time position, with opportunities for professional growth and development.
- Collaborate with multidisciplinary teams to ensure seamless care coordination.
- Develop and maintain effective relationships with patients, families, and healthcare professionals.
Key Responsibilities
- Track all cancer patients who are on stratified follow-up across Lewisham and Greenwich, ensuring investigative scans are being managed and reviewed.
- Raise scan results to the clinician, enabling them to send letters to patients and their GPs in a timely manner.
- Provide support to CNSs with all elements of the recovery package.
Requirements
- Knowledge of medical terminology and strong analytical skills.
- Ability to work independently and prioritise workload efficiently.
- Strong organisational skills, with attention to detail and accuracy.
Salary Range: £32,000 - £42,000 per annum (dependent on experience).
