About the Job

We are seeking a highly skilled midwife to join our team as a Community Midwifery Care Coordinator. The successful candidate will work closely with other healthcare professionals to deliver high-quality care to women with perinatal mental health needs.

About the Role

This is a Band 6 position, and we estimate the salary to be around £38,900 - £44,041 per annum, depending on experience. The successful candidate will be responsible for providing continuity of care (antenatal & postnatal) to a small caseload of women who have been referred to the PMH service and promoting the safety of the child by following Child Protection guidelines.

The ideal candidate will have a broad knowledge of all areas of midwifery and a passion for perinatal mental health and community-based care. They should be flexible, innovative, and resilient with excellent organisational skills and an understanding of mental health conditions that can impact the maternity journey.

Main Duties
  • Provide continuity of care (antenatal & postnatal) to a small caseload of women who have been referred to the PMH service
  • Promote the safety of the child by following Child Protection guidelines
  • Triage referrals to the service by investigating concerns, referring to and liaising with appropriate agencies, and completing appropriate care plans for these women
