Mental Health Specialist

We are seeking a skilled Social Worker to join our Community Mental Health Team as a Band 6 Care Coordinator.
Key responsibilities include:
- Conducting care act assessments and reviews
- Providing safeguarding support
- Assisting with hospital discharge
- Collaborating with the multi-disciplinary team
The ideal candidate will have experience working in a community mental health setting and possess excellent communication skills.
