About the Role

We are seeking a highly skilled and experienced Clinical Triage Practitioner to join our team at Queen Elizabeth's Foundation for Disabled People. As a key member of our rehabilitation service, you will be responsible for assessing and triaging clients with complex neurological conditions, making recommendations for their care and rehabilitation, and working closely with our multidisciplinary team to ensure seamless transitions of care.

Key Responsibilities
  • Conduct thorough assessments of clients with complex neurological conditions, using your expertise and clinical reasoning skills to identify their needs and develop personalized rehabilitation plans.
  • Make recommendations for client care and rehabilitation, taking into account their individual needs and goals.
  • Work closely with our multidisciplinary team to ensure seamless transitions of care, including referrals to other services and agencies.
  • Develop and maintain strong relationships with clients, their families, and carers, providing support and guidance throughout their rehabilitation journey.
  • Stay up-to-date with the latest research and best practices in neurological rehabilitation, applying this knowledge to improve client outcomes and services.
Requirements
  • Registered nurse or postgraduate physiotherapist/occupational therapist with current NMC or HCPC registration.
  • Minimum 5 years of post-qualification clinical experience, with a strong background in neurological rehabilitation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and colleagues.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound clinical decisions.
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality patient care.
