We are seeking a dedicated Adult Nurse Manager to lead our team in providing exceptional care to adults in the community.

This role will involve working closely with a core team of carers, training and supporting them with their clinical skills and activities.

The successful candidate will be responsible for creating personalised care plans for each individual, providing face-to-face support and ongoing monitoring of their physical and physiological needs.

Key responsibilities include designing and delivering tailored care plans, managing medicine delivery, and working closely with community teams and commissioners to establish and maintain bespoke packages of care.

Benefits include 1 day working from home per week, 33 days holiday, private pension scheme, and use of a company car or travel card for community travel.

Requirements:

  • RGN with an active NMC PIN
  • Up to date experience of tracheostomy care and ventilators within the last 2 years
  • Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
  • Full UK Driving Licence desirable but not essential
