About the Role

We are seeking a highly skilled and experienced Community Practitioner to join our Early Intervention in Psychosis team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a Community Practitioner, you will play a vital role in providing high-quality care and support to individuals experiencing their first episode of psychosis.

Key Responsibilities
  • Work as a member of the Multi-disciplinary Team to deliver individualized care that promotes recovery and wellbeing.
  • Conduct comprehensive assessments and develop holistic care plans that address the complex needs of service users.
  • Deliver evidence-based therapeutic interventions, including Structured Clinical Management, CBT, and Family Interventions.
  • Collaborate with other team members to ensure a seamless service user pathway and promote a culture of learning and education.
  • Participate in duty rota and triaging new referrals, ensuring timely and effective response to service user needs.
  • Support health promotion and education initiatives, promoting a culture of wellbeing and recovery.
Person Specification

We are looking for a candidate with:

  • Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Mentor Training.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.
Desirable Qualifications and Experience

We would welcome applications from candidates with:

  • Knowledge of evidence-based treatments for psychosis and non-psychosis.
  • Clinical leadership experience.
  • Experience of working within a community mental health setting.
  • Knowledge of other services that link to community services, including voluntary and statutory agencies.
