Job Summary

We are seeking a highly motivated and compassionate Care Home Assistant Practitioner to join our team. As a key member of our care team, you will play a vital role in providing personalized care and support to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities

  • Assist in the delivery of high-quality care to residents, including personal care, medication management, and mobility support.
  • Work collaboratively with our nursing team to provide comprehensive care and support to residents.
  • Develop and maintain strong relationships with residents, their families, and other healthcare professionals.
  • Participate in care planning and review, ensuring that individualized care plans are in place and regularly reviewed.
  • Contribute to a positive and respectful team culture, promoting a culture of excellence and continuous improvement.

Requirements

  • Care Home Assistant Practitioner Certification
  • Proven ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
  • Flexibility and adaptability, with a willingness to work varied shifts and respond to changing circumstances.
  • A caring and compassionate nature, with a commitment to delivering exceptional care and support.
