Job Summary

We are seeking a highly motivated and compassionate Registered Nurse to join our Community Nursing Team in Torquay. As a key member of our team, you will provide high-quality individualized patient care, support patients' rehabilitation, and promote their independence and quality of life.

Key Responsibilities
  • Deliver patient-centred care to housebound patients in Torquay, working collaboratively with a multi-disciplinary team and in partnership with primary and secondary care.
  • Communicate and liaise effectively with various stakeholders, including GPs, Primary Health and Social Care Teams, Practice staff, Hospital Staff, Community Staff, Zone Managers, Macmillan Nurses, and other relevant organizations.
  • Provide out-of-hours services in collaboration with Devon Docs, SWAST (Ambulance Service), and the Safeguarding Team.
  • Work collaboratively with Nursing and Residential Homes to ensure seamless care delivery.
