About the Role

United Lincolnshire Hospitals NHS Trust is seeking an experienced Children's Community Nurse to join our Lincolnshire Community Children's Nursing Service. As a key member of our team, you will play a vital role in providing high-quality care to children and young people in their own homes.

Key Responsibilities

  • Formulate individual care packages to facilitate smooth transitions from hospital to home.
  • Manage your own caseload and coordinate services to reduce hospital re-admissions and provide support and nursing care to children and young people with acute healthcare needs and long-term conditions at home.
  • Participate in the out-of-hours telephone on-call system to support all children and families on the CCN countywide caseload, enabling admission avoidance.
  • Support the development of ambulatory care and promote the ethos of empowering children and their families to develop the knowledge, skills, and expertise to enable them to become self-caring, independent, and manage their own condition at home wherever possible.

Requirements

  • NMC Registered Nurse (Child)
  • Teaching/mentoring qualification
  • Evidence of continued professional development
  • Ability to travel independently throughout the County
  • Flexible working
  • Commitment to developing children's community services
  • Commitment to participate in a county-wide CCN caseload on-call rota 5pm to 10pm weekdays & 9am to 10pm at weekends and Bank Holidays, including participation in a 24/7 on-call rota for children & young people requiring end of life care.
