Community Support Worker

We are seeking a highly skilled and compassionate Community Support Worker to join our team at Devon Partnership NHS Trust. As a Community Support Worker, you will play a vital role in supporting individuals with mental health needs in their own homes and communities.
Key Responsibilities- Communication and Support
- Communicate effectively with individuals, taking into account any communication difficulties they may have.
- Provide emotional support, advice, and information to individuals, carers, and families.
- Signpost individuals to other services and support networks.
- Community Engagement
- Work with individuals, carers, and families to achieve the objectives of their care plan.
- Encourage social and physical activities as identified within the care plan.
- Promote independence and recovery.
- Collaboration and Partnership
- Foster and maintain good working relationships with community teams, health and social care workers, and inpatient services.
- Share information with GPs, carers, and other agencies to ensure seamless care.
- Record Keeping and Reporting
- Maintain accurate and timely records of all activity.
- Report any changes in an individual's presentation to the care coordinator or appropriate professional.
- Essential
- Awareness of safeguarding principles.
- IT skills.
- Previous employment or experience working with people in a caring situation or previous experience working in a different role within mental health services.
- Desirable
- Knowledge of mental health and a basic understanding of physical health presentations.
- Previous care experience with people who have mental health difficulties.
