Registered Mental Health Nurse

Livewell Southwest CIC is seeking a highly skilled Registered Mental Health Nurse to join our Community Crisis Response Team. As a key member of our multidisciplinary team, you will work closely with patients, carers, and other healthcare professionals to provide holistic care and support in the community.
Key Responsibilities- Assess, diagnose, and implement individualized care plans for patients with complex acute needs.
- Work collaboratively with the multidisciplinary team to provide comprehensive care and support to patients and their carers.
- Develop and maintain strong relationships with patients, carers, and other healthcare professionals to ensure seamless care coordination.
- Contribute to the development and implementation of clinical policies and procedures to ensure high-quality care.
- Participate in reflective practice, clinical supervision, and appraisals to maintain professional expertise and development.
To be successful in this role, you will need:
- Registration with the Nursing and Midwifery Council (NMC).
- Experience working in a community or urgent care setting.
- Strong assessment and communication skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- Commitment to delivering high-quality care and support to patients and their carers.
