About the Role

We are seeking a highly motivated and caring Community Staff Nurse to join our dynamic team in Unplanned Care. As a key member of our multidisciplinary team, you will work closely with patients in their own homes, providing high-quality healthcare and preventing unnecessary hospital admissions.

Key Responsibilities
  • Work collaboratively with the community nursing team to deliver high-quality healthcare to patients in their own homes.
  • Participate in the assessment of health needs and delivery of care to the practice population.
  • Contribute to the prevention of hospital admissions and facilitation of early discharges from hospital.
  • Work agilely across the service, covering a seven-day rota and 24-hour shift pattern.
